Replacement CHL

If you’ve changed your name or address, or if your CHL is lost or stolen, you will need a replacement license. Getting a replacement CHL does not change your expiration date. Here’s what you need to do to replace your CHL…

STEP ONE: Gather and Complete Required Documentation

You will need to bring different documentation depending on whether you are updating your information or replacing a lost or stolen CHL.

Changing Information?

If you are updating information such as a new name or address, bring the following:

  • “Application for License to Carry Concealed Handgun”. This two-page form is part of the CHL Application Packet.
  • Proof of the Change, for example, your driver’s license showing your new name or new address.
  • Current CHL.

Replacing Lost or Stolen CHL?

If your CHL has been lost or stolen, bring in the following:

  • “Application for License to Carry Concealed Handgun”. This two-page form is part of the CHL Application Packet.
  • Photo Identification.

STEP TWO: Schedule an Appointment

Call (541) 766-6606 to schedule your CHL appointment. YOU MUST HAVE A SCHEDULED APPOINTMENT to submit your replacement request.

STEP THREE: Arrive for Your Appointment as Scheduled with Documents and Fees

Location

Come to the Second Floor of the Law Enforcement Building, 180 NW 5th Street, Corvallis, for your appointment.

Fees

The fee for a Replacement CHL is $15.00. You may pay your fee with a check, debit/credit, money order, or cash. Make checks or money orders out to: “Benton County Sheriff’s Office.” If paying in cash, exact change is required.
